Clinical Internship Screening Requirements
Students participating in a clinical internship must complete all required clinical screenings by their assigned deadline. This registration is intended only for CAP students who have been assigned a clinical internship and instructed by Health Science Consulting to complete the required screenings.
10-Panel Urine Drug Screening | $100
Your $100 registration fee covers the 10-panel urine drug screening and processing. After registration, you will receive instructions for scheduling your screening through Labcorp.
Your required Labcorp Chain of Custody form will be provided as a downloadable file after registration. You must print the form and take it with you to your Labcorp appointment, along with a valid form of identification.
Level II FDLE Background Screening & Fingerprinting | Approximately $50-$90 Additional
A Level II FDLE background screening with fingerprinting is also required. This fee is not included in the $100 registration fee and is paid separately to the selected Florida Live Scan vendor.

Check your email immediately after registration. Your order confirmation will include detailed instructions for completing both screenings and access to your required Chain of Custody form.
ALL CLINICAL SCREENING DEADLINES MUST BE ADHERED TO.
Do not wait until the deadline to schedule your appointments. Appointment availability and processing times may vary. Failure to complete the required screenings by the established deadline may affect eligibility to participate in the clinical internship.
